If the 07/17 version is not the latest version then please provide the latest version instead.

Note: I have previous examples of these as two seperate forms, but have been told more than once of people receiving them as a combined form, numbered with the quoted designation, when requested for MR/appeal purposes.

Turns out that it is not a combined form, it's an ESA56 but they have just added the LT54 designation to the new version.
Possibly done just to clarify that it is part of the DMs decision process.
