Description : Number of Referrals from GP Services or Memory Clinics to
the Demential Pathway Service

I acknowledge your request for information received on 2018-07-10

Your request is being considered and you will receive the information
requested within the statutory timescale of 20 working days (Freedom of
Information Act 2000/Environmental Information Regulations) or 1 month
from receipt (Data Protection access requests).

For your information, the Acts and Regulations define a number of

 1. Number of Referrals received across the borough since September 2017

 
301 (GP – 116 referrals since September 2017; Memory Clinic – 185
referrals since September 2017)
 
2. Number of referred cases that currently have no permanent dementia
pathway advisor
 
235 – Of this total 93 memory service referrals and 48 GP referrals have
had input from the Dementia Advisor service, and individuals have had
their need met at point of referral and therefore do not require further
input at this stage of their journey, and are aware they can reconnect
with the Dementia Advisor Service at any time as their needs change.
 

 3. Ratio of qualified dementia pathway advisors to referred patients
(those patients that currently have a permanent advisor allocated to
their case)

 
7 Dementia Advisors actively supporting 33 people referred via GP or
Memory Service. There are dementia advisors allocated to individuals who
have been referred into the Dementia Advisor Service prior to September
2017 and they take referrals from other sources.
