Common Assessment Framework (CAF)

Waiting for an internal review by Birmingham City Council of their handling of this request.

Dear Birmingham City Council,
On 20/10/13 your children services at the Featherstone, Safeguarding Team under a Ms Angela Blake and Team Manager Clair Graham and a Ms Nicola Campbell of Cafcass prepared a CAF on my child having made previous Subject Access Requests on now 3 occasions the reports on the last page have a signed statement of truth that is signed by the Social Worker and the Team Manager on my report sent by your Children services in January 2014 and now on 3 subject access request you have failed to provide me with the missing signed statement of truth that all that is stated in the report is true, please provide this missing page and please provide the letter sent to Ms Nicola Campbell from Practice supervisor Ms Natalie Hardware stating that both parent had been spoken to as set out in the letter from cafcass dated 01/10/13 from MS Nicola Campbell you have failed to provide me this letter on 3 occasions going back to 2014.

Please explain why on 22/11/13 Ms Blake claims to of made a home visit to see my child but there are no notes on record of any such visit ever taking place and there are incorrect dates that conflict with dates of calls made to my child school that are dated after calls made to others that clearly show this report was fabricated and the records have been tampered with and made to suit, this is an act of Fraud under the Fraud Act 2006.

What steps do the council take when Fraud is reported? when children are involved and family due to these worker i lost my child who i have now not seen in nearly 3 years when there report was used to persuade a judge to suspend my contact at at a hearing i was not present at? these workers have emotionally harmed and damaged my child and covered up phone calls and removed data from reports and conspired with cafcass to get the result they wanted not the child wanted.

Yours faithfully,

mark davis
