This request is addressed to your capacity as accountable body for the Leeds Youth Offending Team (or locally-titled equivalent).

I request that you provide me with electronic copies of all minutes, reports and presentations considered by the partnership management board (or locally-titled equivalent) since 1st January 2012.

This specifically includes all documents relating to Local Management Reviews and Serious Case Reviews involving your YOT. I realise that anonymisation will be necessary to remove identifiable service user information from these.

I refer to the above and your email dated 30 September. I have contacted
the relevant services and now provide the following response to your
request for information relating to Local Management Reviews and Serious
Case Reviews.

 

I request that you provide me with electronic copies of all minutes,
reports and presentations considered by the partnership management board
(or locally-titled equivalent) since 1st January 2012.

The YOS does have a partnership board that has oversight and governance of
YOS services.

 

Where there has been a Serious Case Review (undertaken by the LCSB) or an
incident requiring a Local Management Review, a verbal update only is
given at board meetings. The actual reports are classified as RESTRICTED
under the GPMS and are not shared more widely.

 

The attached minutes are for the meetings in 2012 and 2013 give only a
brief summary of the verbal narrative and any lessons learned.

 

The other papers relating to YOS business e.g. finance and performance
reports do not relate to SCRs or LMRs and have not been supplied.

 

The last YOS partnership board meeting was held 11 September 2013 but the
minutes have not yet been published for this.

 

I hope this information is useful to you, however if you are dissatisfied
with this response then you have a right to appeal to an independent
officer within children’s services. On appeal, your request will be
re-examined and an assessment made as to whether the requirements of the
Act have been adhered to. Should you wish to follow this course of action

You also have a right under Section 50 of the Freedom of Information Act

I write in response to your request for an internal review dated 17
October 2013. I have conducted an investigation into this matter and now
provide the following response.

 

Unfortunately due to a misunderstanding we did not supply the full records
as we read your request to be asking solely for the YOT Partnership Board
minutes which related to SCR and LMR. This information was not exempt as
we did not believe the information held related to your request. I have
spoken to the officers concerned and I am satisfied that this was not
intentional and was purely due to misunderstanding the question.

 

Attached are the remainder of the minutes and reports as requested, which
include the minutes dated 05/12/12. Please note any external presentations
or reports brought to the meetings by third party partners are not held by
Leeds City Council.

 

I hope this information is useful to you and resolves your request for an
internal review. I am sorry we did not respond to your satisfaction in our
previous submission. If you remain dissatisfied you have a right under
Section 50 of the Freedom of Information Act to ask the Information
Commissioner to take a decision as to whether we have met with our
