Wrong diagnosis resulting in premature death

Dear Department of Health,

Please could you advise me how many cases of premature death have occurred as a direct result of a wrongful diagnosis in the UK and particularly by the NHS.

I am particularly interested in how many cases have occurred that symptoms have been accredited to 'stress' and 'anxiety' where a more serious or sinister underlying health problem has not been identified.

Any documents you could provide me with regarding mis-diagnosis within the NHS, UK would be of great help.
