Dear NHS England,

FOI England (NHS ENGLAND), NHS England

Dear Mr Burgess,

Re:      Freedom of Information request (Our Ref: SDR - 184752)

Thank you for your Freedom of Information (FOI) request dated 17 December
2013.

Your exact request was,

‘Under the freedom of information act, I'd be grateful if you could
provide a list of GP's who have had their GMS contracts withdrawn since
the 1st Jan 2012.’

NHS England does hold some information relevant to your request.

Please find below a list of General Medical Services GMS contracts that
have been revoked by from 1 January 2012.

·         Woodlands Surgery, Chapeltown Road, Leeds

·         Kingfisher Surgery, Milton Keynes

·         Dr Souymo Gorai, GP at Clements House practice in Knight Street,
South Woodham Ferrers

o Branfill Road Practice, Havering.

·         Dr Gibson, GP at The Blue Practice, Walton Health Centre

Please be aware that from April 2013, NHS England has taken on many of the
functions of the former Primary Care Trusts (PCTs) with regard to the
commissioning of primary care health services. As such, any contracts
withdrawn between 1 January 2012 and 31 March 2013 would have been done so
by one of the former PCTs. The transfer of records from the former PCTs to
the NHS England area teams is not yet complete, and as such we are unable
to definitively confirm that this is an exhaustive list of all revoked GMS
contracts since 1 January 2012.  I can advise however, that the list does
contain all revoked contracts of which NHS England is aware.

We are striving to complete the transfer of legacy data within 6 months of
the current date and believe to that our records management and archiving
systems will be sufficiently developed to enable us to comply with your
request on completion of this process. As such you may wish to resubmit
your request in 6 months time.

Please be aware that although we aim to have completed the transfer of
legacy information within 6 months, we cannot make any assurance that s.12
or any other exemption would not apply to a future request.

I hope this information is helpful. However, if you are dissatisfied, you
have the right to ask for an internal review by writing to us, within two
months of the date of this letter, to:
