By shared care agreements I also mean Joint Memorandum Of Understanding agreements that may serve the same purpose of sharing care or any other equivalent agreement.
I am writing to request information from your institution under the Freedom of Information Act 2000.
My request relates to the Gender Identity Clinic.
I request the following information:
1. Between January and May of this year, how many shared care agreements relating to HRT prescribing were withdrawn or refused from patients using your service?
Can these results be split between refusals and withdrawals that take place after an agreement had originally been accepted.

The Tavistock and Portman NHS Foundation Trust is a small specialist
mental health trust which provides mainly outpatient psychological
services only. The Trust is not a hospital and does not provide acute

Your request for information, as detailed in your email below, has been
handled under the Freedom of Information Act 2000 (FOIA). The Trust’s
response is as follows:
The data you have requested is already publicly available and may be
reached via the following link [1]24-25289

previous request, sent to you on 19/10/24. There you will see, in
Questions 2 and 4, that we have already answered both your questions
relating to HRT prescribing by theTrust, and shared care record data
entries by other agencies regarding their prescribing and/or witholding
HRT.
Section 21(1) of the Freedom of Information Act (FOIA) exempts disclosure
of information that is reasonably accessible by other means, and the terms
of the exemption mean that we do not have to consider whether or not it
would be in the public interest for you to have the information as it is
readily available for public review, as well as having been sent directly
to the email address you provided.
We note that you have refined the request which originally related to all
data between 2020/21 to 2024, this recent request has refined it to
January to May 2024. Although the amount of data will have reduced this
will still involve a large number of cases, therefore the response remains
the same in that ‘we can confirm that this is a frequent occurrence, which
would be entered as free text within the notes sections of a patient’s
electronic record. This would not be in an electronically retrievable
field format, (ie there is no automated mechanism for the collation of the
required data). To extract this data, where held, would require manual
review of all patient records, amounting to thousands of electronic
patient records, which, at an estimated 15 minutes per record would take
approximately 250 hours per thousand records.
This means that as the time required to process this question falls
outside the cost limit, and is refused and exempt from disclosure under
section 12 of FOIA.

held by this Trust at the date on which the request was made.
We hope that you are satisfied with this response. If you are
dissatisfied you can ask us to carry out an internal review of our
