Who in the DWP oversees and actions failings in the private companies carrying out benefit assessments

The request was partially successful.

Dear Department for Work and Pensions,

There have been many failings reported about poor quality assessment reports from Atos, Capita, Maximus who in the DWP is responsible for overseeing the process?
Who is accountable when someone is wrongly found fit for work? DWP decision maker or the private company who carried out the assessment?

Yours faithfully,

Trudy Baddams
