Dear South Western Ambulance Service NHS Foundation Trust,
Dear Kevin Pointer / Clare Lunk

In November 2018 Kevin Pointer kindly pointed out:

"The process for the management of commissioned investigation reports is that once a final version has been agreed, any previous drafts are destroyed to ensure only the approved version is retained and actioned" Kevin Pointer, SWASFT Information Governance Manager 2018.

I understand SWASFT is a Policy lead service and would therefore be grateful if Kevin Pointer or his assistant Clare Lunk could please highlight to me, which policy and subsection of that policy allows for this process of deleting the original commissioned report in favour of SWASFT's own 'interpretation' of that report.

Dear South Western Ambulance Service NHS Foundation Trust including Clare Lunk.

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of South Western Ambulance Service NHS Foundation Trust's handling of my FOI request 'Which policy allows deletion of original investigation reports.'.

It is becoming increasingly apparent that SWASFT acts with impunity choosing which policies it will abide by and which it will ignore - to its own advantage. As outlined in Prof Lewis's 'Cultural Review' of October 2018.

This appears to be exactly one of those instances where Mrs Beet cannot give a specific response which is likely to be satisfactory, so has said "she is happy and therefore the matter is closed".

I ask again, which policy allows SWASFT the luxury of re-writing independently commissioned reports and then deleting previous drafts and also the original independent report?

If Mrs Beet cannot supply such information the inference is quite clear, and further damaging to the already tarnished reputation of SWASFT, Ken Wenman and Mrs Beet as senior HR representative.

In addition, I will add this question to a growing list of questions which will be put to Mr Wenman in Westminster in the near future.

I feel sure Mr Wenman would wish to try and uphold the edict of Truthful, Honesty and Transparency within SWASFT and not hide behind dismissive one liners in order to protect what some would perceive as wrong doing.
