Dear Parliamentary and Health Service Ombudsman,

I have recently asked for information about me. You have replied it is destroyed. Therefore there was information or it could not have been destroyed.

Please tell me on what date was my information destroyed?
Who would have trigger this deposal?
Is information that is destroyed certificated as is requiried when NHS destroy records?

You have also given the date of the last SAR that was given to me. This SAR followed on consecutively from pervious ones, and yet there is no reference to any meetings that are referred to in Dame Julie's diary.

Why is that?

Clearly when there is a meeting to 'prepare' for a meeting with me and my MP there would be notes and yet none have been given to me. Clearly if it was not so secret that The Dame held a meetings behind people's backs, about meeting complainants and their MP's, and I had known, I would have asked why it as not in the SAR's received from you.

But I could not have known this as it was unknown to me. I therefore relied on the integrity of PHSO to uphold the law on Information Rights correctly.

Why was this information not released to me?

I have been told I have all information I am entitled to going back many years. I'm told it is consecutive...And yet nothing re your preparation meeting...

Has anyone in PHSO considered whether this destruction policy, I am guessing it is policy, should be reconsidered?

Or is it that the policy serves PHSO's best interest?

Yours faithfully,

Brenda Prentice

Dear Parliamentary and Health Service Ombudsman,

Thank you for your reply that came by post.

You have, in the usual fashion of PHSO, answered questions I didn't ask and not answered questions I did ask, such as :

Why was the information I am asking for, not given to me when I last asked for a SAR? To tell me it is now destroyed is no answer, why was it not given to me when I asked for it the first time?

It is not 'information held beyond that date', it is why the information was not given to before that date, when I last aske for a SAR?

There is no point in telling me your retention policy is being reviewed, that does not answer my question.

AND I am not asking for opinions! I just want to know why it appears PSHO have not given information to me that it DID hold.

You hope the information is helpful!

Again PHSO have looked at the process and not the out come, which seems to me is, PHSO withheld information I was entitled to see.

Do you intend to address this?

Yours faithfully,

Brenda Prentice
