Many thanks for your prompt acknowledgment and on reflection it may be helpful in I fully clarify my request:

Since formation of the Trust :

1) No of inpatient deaths per year
2) No of inpatient deaths occurring at BH and weekends each year
3) No of out-patient deaths (people subject to care in the
community on Section 117) per year
4) No of out-patient deaths occurring at Bank Holidays and weekends
each year
5) Total number of patients each year (in and out patients)

I believe 'unexpected deaths' as go before the coroner.... yes? But excludes people who have a mental health condition but die of natural causes!

Dear Julie Benfell,
Not pedantic at all, thank you for taking time to consider my feelings.
I too hope this sounds ok for you.....its a difficult issue to broach.
But there must be a more definite definition than "when we do not expect the patient to die." I could use the usual misunderstanding of "they are just going to do it anyway".....therefore is that then considered an 'expectation?'
Do you see my quandary?

Apologies for the delay in answering your email this was due to annual
leave.

Definition of an unexpected death would be when a person dies and the
cause of death has not been established, and is only established once
the Coroner has determined that cause.

Dear Barbara Appleby,

Thank you for this and my question seems to be causing some confusion, my apologies. Having thought about specifics, I hope this will clarify everything:

1)Re CAMHS:
a)Number of out-patient deaths each year out of the total of out-patients treated?
b) Number of in-patient deaths each year out of total number of in-patients treated?
c)If possible I would also like to access the number of deaths in each category that occurred on Bank Holidays or at a weekend?

2) Group 2....Same a- c list for Working age Mental Health
Services?

3) Group 3....Same a-c list for Older Persons services?

Could I advise that I understand I am looking where possible to eliminate death by natural causes which I believe may particulary affect Group 3.

Therefore, if its helpful, please leave out deaths by natural causes or where the Coroner has ruled natural causes.

Dear Ms Speers,

You are quite right in stating that your original request was due
yesterday (22 June 2010), however you did respond back to the Trust with
further clarification of your request, and in accordance with the act
the 20 working days starts from when the Trust received that further
clarification.

However I have chased the department this morning to get the information
to fulfil your request and the Trust will respond to you as soon as we
have that information.
