Was approval granted for Concordia's price rises of Liothyronine Thyroid med?

The request was refused by Department of Health and Social Care.

Dear Department of Health and Social Care,

Concordia (now known as Advanz)are currently under investigation by the CMA for the price inflation of liothyronine (T3), a medication for hypothyroidism. Over the course of 10 years, Concordia incrementally increased the price of T3 by 6000%. The CMA investigation relates to the period during which Concordia raised the price by 1600%

The information I am requesting is:

1. Did the Department of Health (DoH) give approval for these price increases? If so, please supply the document which confirms this.

2. Did the DoH not specifically approve these price rises, but fail to object to them?

3. Which department within the DoH would have been responsible for any decision to object?
Who currently heads this department?
