I don't know what I can do about the form, it was attached to my email, so it appears to be a problem with WDTK; I have had this happen before. I'll attach it again and hope their system can cope with a single attachment.

You asked for 'the format of a warrant of committal for non payment of council tax' under the FOIA, and therefore I sent you information in the possession of MOJ, i.e. the format used by its operating system to produce the warrant. That is all that can be provided under FOIA; explanations, justifications etc are outside its remit.
