Warning Markers
Dear Leicestershire Partnership NHS Trust,
1. Do you operate Warning Markers or other alert systems in order to log that a service user is aggressive, non-compliant, etc. or has a history of a certain behaviour known to the Trust?
2. Can you please provide an anonymised or redacted visual demonstration of how this information is displayed to service providers and staff?
3. What are the categories and types of behaviour that warrant being placed on such a system? Is there a specific list of categories or can you add any reason?
4. Do you inform service users of notifications placed on their digital record or that they are on a Warning Marker system
5. When and how are these lists added to and reviewed and by whom? Please provide their names and email addresses.
