Thank you for your information request of 18 August 2017. You requested
the information about whether incidents had been reported to MAIB, and if
so, the date of submission of the reports, name of the reporting person
and any follow up action taken or outstanding.
Your request has been considered under the Freedom of Information Act
2000. I am writing to confirm that the Marine Accident Investigation
Branch (MAIB) of the Department for Transport has now completed its search
for the information.
Please find below a table with relevant info about the incidents we found
in our database records. MAIB did not investigate any of these
incidents. We record only the type of body that first reported an
incident to us. It is possible that the name of the person who reported
an occurrence may be in our paper records, however I believe that section
40(2) of the Freedom of Information Act exempts us from providing this
information to you. A couple of the incidents listed have very different
dates to those you provided, they may not be the same incidents.

Thank you for your e-mail of 29 September 2017 asking for a review of the answer you were sent on 18 September 2017.
I can confirm that I have looked at your original request and the handling of the answer that was prepared.
In your request you gave details of a date, vessel name, and accident type for 77 separate occurrences. You asked whether we had received a report in respect of each of these, and whether we could provide the name of the reporting person and whether any follow-up action had been taken or was outstanding.
For clarification, the list of eight occurrences sent to you by Cathy Pennock on 18 September were the only ones of the 77 you listed for which the MAIB have any record.
