Type II Diabetes Education Programmes

Information Act 2000. I am outlining my query as specifically as possible.

I would like to understand the Trust’s provision of Type II Diabetes Education Programmes, specifically those commissioned through your GP practices and consortia. Where possible, I would be interested to know the number of current and previous patients supported, the name of the chosen education programme(s), and the number of surgeries involved.

I am interested in any information held by you regarding my
request. I understand that under the Act, I do not have to specify particular files or documents and that it is your department's responsibility to provide the information I require. If you need further clarification, please contact me by email.

Please find below the information that you requested from North East
Lincolnshire Care Trust Plus under the Freedom of Information Act.

 

This trust currently provides the DESMOND programme for NEWLY DIAGNOSED
Type 2 patients only.  This has been provided since June 2006, and has
been commissioned by the PCT and provided by the Community Diabetes Nurse
Specialist with assistance from a Dietician from NLAG (Northern
Lincolnshire and Goole Hospitals).

On average there are 2 courses per month with a maximum 12 patient per
two-part course.

All surgeries within the boundaries of NELCTP are able to refer patients,
and all barring one do refer.

 

Figures;

 

Exact figures have only been kept since 2008,

Since June 2006 – there have been 1403 referred (average of 18 per month)

Numbers of patients ACTUALLY completing this course as follows

 

2008/9  = 162

2009/10 = 137

2010/11 = 172

March 2011 to Oct 2011= 81

 

Currently waiting (either to be appointed or start a course) = 77
