Dear Lincolnshire Teaching Primary Care Trust (PCT),

I am writing to request information under the Freedom of
Information Act 2000. I am outlining my query as specifically as possible.

I would like to understand the Trust’s provision of Type II Diabetes Education Programmes, specifically those commissioned through your GP practices and consortia. Where possible, I would be interested to know the number of current and previous patients supported, the name of the chosen education programme(s), and the number of surgeries involved.

I am interested in any information held by you regarding my
request. I understand that under the Act, I do not have to specify particular files or documents and that it is your department's responsibility to provide the information I require. If you need further clarification, please contact me by email.

FREEDOM OF INFORMATION – DECISION NOTICE

 

Dear Madam,

 

Diabetes Type II Education Programmes (ref 980)

 

I refer to your email of 22 November requesting information in respect of
Diabetes Type II Education Programmes.

 

I can confirm in accordance with S.1 (1) of the Freedom of Information Act
2000 (FOIA) that we do hold the information that you have requested. I
shall address each of your questions in turn:-

 

The Trust’s provision of Type II Diabetes Education Programmes,
specifically those commissioned through your GP practices and consortia.

 

-                      NHS Lincolnshire provides diabetes education as
follows:

o    For newly diagnosed Type 2 patients access to Spotlight training is
available across the County – all Practices can refer Patients to this
service

o    Work is on-going to provide structured follow up education for
existing Type 2 patients

o    The service is provided via Lincolnshire Community Health Services
and is based up similar national programmes of education

o    Work is also taking place to ensure the local service complies with
National Standards for Diabetes Education

 

-          Type 1 Education is provided by United Lincolnshire Hospital
Trust’s on all three main sites – Lincoln, Boston and Grantham

-          We do not hold information regarding any diabetes education
which may be provided separately by individual Practices.

 

 

Where possible, I would be interested to know the number of current and
previous patients supported, the name of the chosen education
programme(s),

and the number of surgeries involved

 

1st Jan 2011 to 16^th Dec 2011 1,477 patients were referred for Diabetes
education.

356 did not respond to invite, did not attend first session or did not
attend the second session. Please note that that it is the GP’s and
practice nurses who refer patients to the Spotlight programme.
