Type II Diabetes Education Programmes

I am writing to request information under the Freedom of
Information Act 2000. I am outlining my query as specifically as possible.

I would like to understand the Trust’s provision of Type II Diabetes Education Programmes, specifically those commissioned through your GP practices and consortia. Where possible, I would be interested to know the number of current and previous patients supported, the name of the chosen education programme(s), and the number of surgeries involved.

I am interested in any information held by you regarding my
request. I understand that under the Act, I do not have to specify particular files or documents and that it is your department's responsibility to provide the information I require. If you need further clarification, please contact me by email.

I would like to receive the information as electronic copy, via WhatDoTheyKnow.com.

If my request is denied in whole or part, I ask that you cite one of the Freedom of Information Act exemptions for each piece of information you refuse to release. On 22 November you sent a request, made under the Freedom of Information
Act, to both NHS Dorset and NHS Bournemouth and Poole. 

 

From 01 July both organisations, named above, became the Pan Dorset
Cluster with an overall Chief Executive and Board.  I am, therefore,
responding to your request on behalf of the Pan Dorset Cluster and now
provide the following information:

 

1.         Both NHS Dorset and NHS Bournemouth and Poole commission type 2
diabetes education programmes for newly diagnosed patients with type 2
diabetes.

 

2.         As part of the NHS Annual Operating Plan 2012/13 a review of
all diabetes education provision is in progress with a proposed completion
date 31 March 2012.

 

3.         There are currently two main providers of specifically
commissioned type 2 diabetes programmes within the Pan Dorset Cluster;
they are Dorset Healthcare University Foundation Trust (DHUFT) and Dorset
County Hospital NHS Foundation Trust (DCH).  Three different courses are
delivered:

o   DESMOND; provided by DHUFT.  They run 10 per year with 10-12 patients,
plus carers, per course.  100-120 places

o   Initial Education programme for type 2 Diabetic patients provided by
DHUFT.  These are weekly courses.  New patients figures this financial
year to date is 1218(extrapolated)  and total contacts to the programme
(three part course) is in the region of 3594.

o   DES provided by DCH.  300 places offered per year, with monthly
sessions.

o   Provision of services reflects the numbers of newly diagnosed per
month and the rural/urban nature of the area.

 

4.         There is a recognition that not everyone wishes to, or is able
to, attend a group education session so GP practices offer individual
education to patients.

 

5.         In terms of type 2 diabetes on-going education.  This is
normally delivered by GP practices on an individual basis.

 

6.         Denmark Road Surgery, Poole, holds group education as well as
services mentioned in point 5 (above).  GP practices in Bridport (70
attendees) and Weymouth (30 attendees) have worked with Diabetes UK to
deliver group education sessions during this financial year.  These are
likely to be repeated.
