Dear Hugh
Thank you for your email which we received on the 25 September 2014 regarding copies of requests or written submissions in relation to the health review Transforming Your Care.
Your request has been dealt with under the Freedom of Information Act 2000.
Our records would indicate that there have been no further meetings or correspondence with the health service since my reply, dated 24 August 2012, to your previous FOI request. However, for the sake of completeness I have attached the correspondence (which you have previously received) some of which falls within the timeframe requested.
I trust that you will find the response satisfies the duty of disclosure imposed by the Freedom of Information Act 2000. However, I would advise you that if you are unhappy with the response or if you are unhappy with the way we have handled your request you have the right to seek a review of this matter by writing to:

Dear Maria Dale,
Many thanks for the reply, however none of these documents relate to the health review "Transforming Your Care".
The attached documents all relate to a time period that pre-dates the creation of Transforming Your Care.
it would be my understanding that these documents relate to the previous health consultation named the Comprehensive Spending Review.
If there are no documents that relate to Transforming Your Care can you update the response to note that there are none rather than an answer that is not a true reflection of the question posed
Yours sincerely,
hugh mc cloy
