TV Licensing Data Security

Dear British Broadcasting Corporation,

I am making this request for information under the terms of the Freedom of Information Act 2000.

Please provide the following information held by the BBC since 1st January 2014:
1. A list of incidents where the personal data of TV Licensing "customers" has been subject to loss, damage or unauthorised or unlawful processing. I am aware that Capita is contractually obliged to report all such incidents to the BBC's Head of Revenue Management.
2. For each incident disclosed in response to point 1 above:
(a) The date of the incident (e.g. when it occurred or was discovered).
(b) A description of the personal data involved (e.g. customer names, bank account details, addresses etc.)
(c) The number of customers whose personal data was involved.
(d) Brief circumstances of the incident (e.g. personal data was mistakenly emailed to an unauthorised third-party, left on public transport etc).
(e) Whether or not the incident was reported to the Information Commissioners' Office and/or customers concerned.

By "customer" I mean any member of the public TV Licensing has dealings with, irrespective of their legal need or otherwise for a TV licence.

In accordance with section 11 of the Act, I require your response to be delivered in electronic format to the return email address associated with this request.
