Transition from CAMHS to Adult Services in Salisbury
Dear Oxford Health NHS Foundation Trust,
Please provide your process, policy, procedure or similar information that describes the steps your staff will take when transitioning a young person from your care to AWP.
In addition, please include information from your management system that demonstrates what level of interest CAMHS retains in the care of the young person once they move from your care to the care of AWP? Please indicate how this represents transition as opposed to hand over.

Thank you for your email requesting information regarding transition from
CAMHS to Adult Services in Salisbury.
We are treating your correspondence as a request for information under the
Freedom of Information Act 2000.
You should normally expect to receive a further reply from us within 20
working days from the day when your request was originally received by
this Department.
The Trust will endeavour to process your request. However, in light of
the national emergency and the Trust prioritising Covid-19 staff time is
at a premium to provide and support clinical services and we may not be
able to provide a timely response.

I would be grateful if I could also have the previous version(s) of:
Transfer and Transition of Patients and their Care between Services and Providers CP47, so I can read the processes which were in operation and vallid throughout 2018-2020.
I note there is roughly a 6month gap between the date of CP47 and it being signed. Please can you tell me on which date it became the new way of operating?
