Transfer of GP records prior to April 2013

The request was partially successful.

Dear NHS England,

Please confirm how a patient's GP medical records would have been transferred to a new GP practice prior to April 2013 in Bracknell - Berkshire?

Also, what would be actually be transferred to the new GP Practice? Would all computer records that refer to consultations that had been held with the former GP Practice be transferred to the new Practice?

If not, please explain how the new GP Practice would have a full medical history of the transferring patient if such information was not transferred across to the new Surgery.

If the former GP Practice's computer records on 'all consultations that had been held' while the patient was with them are not transferred to the new GP Practice, then what happens to those former medical records - surely they are not held by the former GP Practice, when that medical practice is no longer the transferring patient's GP?
