Dear Department for Work and Pensions,

Thank you let me clarify.
I have requested any training manuals, guidance and material provided to health
professionals (HP)s at Capita for evaluating a Personal Independent Payment (PIP)
application and assessment reports.
In the second part of my request, I have asked for any training manuals, guidance and
material to include handouts provided to Capita when inputting a decision on and recording
their findings on an applicant after a telephone or face to face consultation.
I can clarify my request refers to any training manuals, guidance and
material for HPs undertaking PIP assessments by telephone or face-to-face.this can include any slides and./ or training manuals to include role plays and scenarios plus tests such as cognitive and memory

Yours faithfully,

David Andrew Jameson

request specifying which condition(s) I would like information on.
Conditions:
Autistic Spectrum Disorder
ADHD

Dyslexia
IBS / Irtitable bowel syndome
Kidney Disease
Rheumatoid Arthritis

PTSD

If this exceeds costs place priority on the order listed

Yours faithfully,

David Andrew Jameson
