The request was partially successful.

Dear Transport for London,

Minute No. 46/09/17 from 22 November 2017 Safety Sustainability and Human Resources [SSHR] Panel Actions List (cf. http://content.tfl.gov.uk/sshrp-20171122...) states that then- TfL Surface Transport MD Leon Daniels was responsible for providing SSHR Panel Members with a “timeline of the fatigue management audit work carried out on First Group”. Can you confirm the date when this timeline was provided to SSHR Panel Members? Please provide me a copy of this timeline.

Yours faithfully,

I can confirm we do hold the information you require.

The fatigue management audit fieldwork started on 13 June 2017 and was
completed on 15 June 2017, in response to concerns about driver fatigue
following an incident recorded by a member of the public on a tram in
Church Street, Croydon in May 2017, and was separate to the Sandilands
investigation. This timeline was set out in the Audit Report, which was
issued on 15 September 2017 and provided to the Safety Sustainability and
Human Resources (SSHR) Panel Members on 22 November 2017. This is
published on our website on the SSHR page. A copy of the report is
available via the following link:
