Please could I have the following information.

The total number of in-patients registered as having dementia (please include all forms of dementia and irrespective of whether it is one of multiple diagnoses or the sole diagnosis) who have been referred to a care home or a nursing home over the following years:

• 2011/12 Total number of registered dementia inpatients referred to a care home or a nursing home
• 2012/13 Total number of registered dementia inpatients referred to a care home or a nursing home
• 2013/14 Total number of registered dementia inpatients referred to a care home or a nursing home Year To Date (YTD)

Please can you break down the data by care home and nursing home and also provide the address and post code for each care home and nursing home

Further to your email dated 21^st June 2014, I can advise of the
following.

Please can you break down the data by care home and nursing home and also
provide the address and post code for each care home and nursing home
 
We are unable to provide this information, as the data would not be
centrally held but would form part of the patient’s health record. To
perform a manual exercise to identify, locate, extract and collate
relevant information is estimated to exceed the £450 compliance limit. As
such under Section 12 of the Act, we are not obliged to provide the
information to you. We can however comply with our duty to confirm or deny
whether we hold the information, by confirming we do hold data from which
the requested information may be collated.

Dear Mr Abrams

 

Regarding your amended request below, we have gone back to the
Pre-Discharge Team to ask if we are able to supply data restricted to
2013/14 and this is their response:

 

Once the HNAs DSTs with all information has gone to the respective Social
Services and patients placed into homes with dementia we do not keep a
trail of who went where and what diagnosis.

 

All relevant Social Services would have to keep a record of who they place
in homes and why.

 

As for BHRUT our records are maintained in the actual patients' records so
a huge audit of every patient placed into a residential or nursing home
would need to be pulled to get this information.

 

In view of the information above we can confirm that we are unable to
respond to your request, even with a smaller time frame, within the
compliance limit as per our original response dated 16th July 2014.

 

If we can be of assistance in the future please do not hesitate to get in
touch.
