Dear Akinwamide Mojisola (Digital & Information),
Thank you for your reply. After reviewing our initial request, we recognise that it may be challenging to provide answers to some of the questions we have put forward.
With that in mind, we would like to withdraw the original request and instead submit the below request, which covers similar topics but will hopefully be easier to provide information for. We recognise that this may be treated as a new FOI and so the response process will likely start over - please can you confirm the estimated response date and let us know the reference code for the second FOI.

- Yes
- No
2. Has there been any case in the past 12 months where a HAI has resulted in the death of a patient at City Hospital?
- Yes
- No
3. What is the most common type of Healthcare-Associated Infection (HAI) recorded at City Hospital?
4. How many cases of the HAI identified in the answer to question 3 has City Hospital recorded over the past three years?
5. What specific infection control policies and procedures are currently in place to prevent HAIs?
6. How often are these policies and procedures reviewed and updated at City Hospital (please provide dates of the last review/update period)?
7. Are there any specific protocols in place to deal with an outbreak of HAIs at City Hospital? And have there been any recorded ‘outbreaks’ of HAIs at City Hospital in the last three years?
8. Have any new technologies or practices have been implemented to reduce the rates of HAIs at City Hospital over the past three years (please give details where possible)?
9. Are there any plans to explore or introduce new technologies or practices at City Hospital to reduce the rates of HAIs over the next three years (please give details where possible)?
10. Has City Hospital made the decision to use single-use medical equipment in place of reusable options either fully or partially because of benefits to patient safety/infection control?
