Termination Payments

Waiting for an internal review by West London NHS Trust of their handling of this request.

Dear Sir or Madam,

Please can I have information on the 'termination payments' paid to Mr Simon Crawford and Mr Grant Macdonald on their resignation from their respective posts of CEO and Nursing Director at West London Mental Health Trust?

Yours faithfully,

D.Speers

Dear West London Mental Health NHS Trust,
A response to this FOI request is now overdue. I understand the Information Commissioner has written to you outlining the requirements of the FOI Act and has clearly stated: "Any public authority in receipt of such a request is under a duty to respond within 20 working days of receipt. You should state whether or not the information is held in a recorded form. If it is held, you should either provide the information or issue a refusal notice in accordance with the requirements of section 17 of the Freedom of Information Act or regulation 14 of the Environmental Information Regulations as appropriate.

You can find more information on refusal notices contained in the guidance issued by the Commissioner which is available at:

Dear West London Mental Health NHS Trust,

A response to this FOI request is now overdue and you are breaking the law. As WLMHT have of late, consistently refused to comply with the requirements of the FOIA, I believe the Information Commissioner has written to you outlining the requirements of the FOI Act and has clearly stated: "Any public
authority in receipt of such a request is under a duty to respond within 20 working days of receipt. You should state whether or not the information is held in a recorded form. If it is held, you should either provide the information or issue a refusal notice in
accordance with the requirements of section 17 of the Freedom of Information Act or regulation 14 of the Environmental Information Regulations as appropriate.

You can find more information on refusal notices contained in the guidance issued by the Commissioner which is available at:

Please now respond to any outstanding questions or I shall be evoking the delayed response element showing as ‘withdrawn’ on ICO records.

Yours faithfully,

D. Speers

Dear Dr Patrick,

As advised I have had this link checked by WDTK technical department and they could not find anything wrong.Could I suggest that maybe the fault is with WLMHT equipment?

For ease of reference I am re-sending my most recent FOI request and as you will see this was requested on 5th November. As the time has already elapsed I believe a prompt reply is a reasonable expectation.

Kind regards and I will stagger my FOI requests to help with information access.

Yours faithfully,

D. Speers

Dear West London Mental Health NHS Trust,

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of West London Mental Health NHS Trust's handling of my FOI request 'Termination Payments'.

Unless this FOI request is acknowledged within 24 hours I shall be making a formal complaint to the ICO about WLMHT handling of my FOI requests. There has been sporadic communication from WLMHT (usually when reminded of requirements of FOI Act by ICO) in the last six months and nobody will explain why?

A full history of my FOI request and all correspondence is available on the Internet at this address:

Yours faithfully,

Dear Dr Patrick,

I believe WDTK and WLMHT have sorted out the technical problem and I'm grateful for both for your time.

As WDTK have indicated, they intended to identify and resend all outstanding FOI requests and this is one that has been outstanding since "5 November 2009" I accept that you didn't receive this and am just querying if the message left above is an acknowledgment of receipt or do you need this resent again?

Please advise accordingly.
With thanks

Yours sincerely,

D. Speers
