I am writing to make an open government request for all the information to which I am entitled under the 1994 Code of Practice on Access to Government Information. In order to assist you with this request, I am outlining my query as specifically as possible. As you will understand, this may not be a definitive list of relevant information.

I refer to the publically available Report entitled,

"Clackmannanshire Integrated Mental Health Service
REPORT ON ISSUES AND COMPLAINTS 2010-11"

Please will you provide further details, regarding the annual survey described in Section 3b of this report, concerning the "Service User Survey".

(1) What is the total number of returned and completed survey request documents issued to the random selection of 150 Service Users of Clackmannanshire Integrated Mental Health Service?

(2) Out of these returned survey requests, please provide the number of returned survey documents which were;

(a) Used to provide statistical information in particular for Section 3b, "A low percentage of service users agreed with the following statements: I have seen a written assessment of my needs completed by mental health services (34%). I have been offered a clear plan that describes the services I will receive (66%). At least once a year there is a review to discuss the services I receive (48%)"?

(b) The number of above stated responses, of the 150 Service Users contacted, which were unclear or incorrectly completed leading to such responses not being included in the survey?

(3) Out of the responses received from the survey of 150 Service Users, please provide numbers of returned and completed requests completed by a third party and/or carers?

(4) Out of the responses received from the survey of 150 Service Users, please provide numbers of returned but uncompleted if any?

I would be interested in any information within your organisation regarding my request. I understand that I do not have to specify particular files or documents and that it is the department's responsibility to provide the information I require.

I understand that under the act, I should be entitled to a response within 20 working days. I would be grateful if you could confirm in writing that you have received this request. I look forward to hearing from you in the near future.

Further to your request for information in respect to the Clackmannanshire
Integrated Mental Health Service Complaints and Issues Report 2010-2011, I
refer to you the report entitled Clackmannanshire Integrated Mental Health
Service Annual Report 2010-2011; in terms of your specific enquiry details
can be found on page 13 entitled Evaluations.

However, you may find the following points summarised below helpful with
regards to your enquiry.

1 Out of the 150 surveys sent out, 43 (29%) were received.

2a 43 were used to provide the statistical info but 2 people selected not
applicable and 2 people did not answer the question.

2b 43 were used to provide the statistical info but 1 person selected not
applicable and 1 person did not answer the question.

3 Responses received were anonymous and therefore it is not possible to
comment on whether responses had been completed by a third party and/or
carer

4 43 responses were received and all responses were utilised.
