I am writing asking for your help with some information around your use of telehealth, smartphone 'Apps' , and remote health coaching for patients with long term conditions:
including, but not limited to: COPD, Asthma, Diabetes and/or Heart conditions.
1. Do you have any of the above in use in your services at the moment, that have completed a pilot phase and are being rolled out, or used operationally?
1a If you do, what systems are you using?
Then, for each system;
1b If you do, how many patients, by condition, are you supporting, and what % is this of the number of patients you eventually plan to support
1c If you do, please share with me any benefits analysis undertaken around non-conveyance or other clinical or commissioning benefits, or point me to where these have been published
1d Please share with me any information you can around the internal business costs of this service, what clinical teams support the service, how are they structured, how many staff, what roles have they got, how were they trained, is there a 24/7 element to the service
1e Please share with me how the patients are supported to use the service
1f Please share with me how any telehealth devices are supported
2. If you are running a pilot, please describe it using the questions above
3. If you have run a pilot in the past that was un-successful, please may I see any lessons learned reports, project closure reports and/or evaluation documentation.
