How many people received intermediate care or reablement services (short term support on leaving hospital)?

How much was spent on care packages for people leaving hospital as part of their discharge arrangements?

How many people received a social work needs assessment whilst an inpatient?

How many qualified social workers working within hospitals does the local authority employ?

I am writing to confirm that the Adult Social Care has now completed its search for the information which you requested and is outlined below:

In relation to the requests made:

1. Is not clear what is meant by social work support on discharge. We have taken this to mean number of patients who received commissioned care (including intermediate care) on discharge rather than simply number assessed by a social worker which forms part of request number 5.

2. We cannot easily provide a breakdown of primary health conditions as this is not recorded electronically and would require us to individually interrogate all assessments completed (in access of 1200).

3. We have taken this to mean intermediate care provided by social services rather than health and to include re-ablement services and residential rehab.

4. Unclear whether this request is to include dom care and residential care. We provide info broken down between private dom care, residential and intermediate care services.

5. Numbers receiving social work assessment - 712 people, 800 provisions, 1450 assessments carried out on 931 people.

6. During 2009/2010 the number of qualified social workers in the hospital team has varied according to vacancies etc. but the establishment figure was 8 + 1 specialising in intermediate care.

Not all patients are assessed by the ASC team (a limited number are seen by community social workers) but we are assuming that the information requested does just relate to patients dealt with by ASC team i.e. "social workers working out of local hospitals".
