Dear Dudley and Walsall Mental Health Partnership NHS Trust,
Please can you supply me with numbers of people who have taken their own life:
1. Following referral to Access team, and while waiting for assessment or intervention
2. Within one week following assessment by Access or Crisis team.
Please supply statistics for the past ten years.
FOI 19-436 - request regarding suicides
Thank you for your recent Freedom of Information request we are now investigating the nature of the search and the quantity of information requested. Under the terms of the Freedom of Information Act we will expect to respond to this request in 20 working days.
Please be aware there are a number of exemptions within the Act which could stop the release of information. The information will be assessed for these exemptions prior to us releasing the information to you. We will advise you if we are unable to provide the information requested and will confirm the exemption used.
In addition to this, please note that there may be a cost associated to your request in line with section 12 of the Act. If there is a cost associated with your request we will contact you and explain the cost.
In accordance with section 12 of the Freedom of Information Act 2000 where multiple requests for information are received from one person or by different persons who appear to be acting in together in a single campaign the estimated cost of complying with any of the requests is to be taken to be the estimated total cost of complying with all of them. To this effect your requests for information will be dealt with under the single reference number above.

FOI 19-436 request regarding suicides
We have reviewed the information which may be relevant to your Request for
Information. However the time taken to locate, retrieve and extract the
information requested exceeds the 18½ hours work and the appropriate fees
The information is not recorded within our systems in a way to pull the
information which you require, it is also not in an easily accessible
format to enable the information to be gathered within the appropriate
timescales. The Trust has on average 50 Serious Incidents per year and
each of these would need to be reviewed to determine if these are linked
to suicides, further to this there may be a requirement to review the
patient records if the details that you have included within your request
are not included within the initial serious incident report. The Trust
anticipates that to retrieve the information would take a minimum of 110
hours which equates to approximately 15 minutes per serious incident to
review the report and gain information from this; this calculation does
not include the time that may need to be taken to review information which
may be contained within the health record. Due to this the Trust is
applying an exemption due to the request exceeding the appropriate
The Trust is regarding this response as a full and complete discharge of
its responsibilities under the Freedom of Information Act 2000 in relation
to this request.
