I am requesting information as per the following, the past 5 year suicide rate within the NHSCT;

1] Suicide rates for inpatients from 2007 - 2011.
2] Suicide rates for outpatients from 2007 - 2011.
3] Demographic data for these suicides (age, ethnicity, gender,diagnosis of psychiatric condition, previous suicide attempts, risk factors for suicide, treatment)
