Dear South Staffordshire and Shropshire Healthcare NHS Foundation Trust,
This is a FOI request for general information about your Trust's awareness and handling of Subject Access Requests. Note that this is *not* a Subject Access Request. Thank-you.
Subject Access Requests in general.
1. Please state the number of Subject Access Requests received by your Trust in the past three years thus far, divided into years.
2. Of the total SAR received in the past three years thus far:
(a) What percentage of SARs were made by the Subject in person?
(b) What percentage were made by legal or other representatives of the Subject themselves?
3. What percentage of the total SARs received by your Trust to date, were complied with within the forty-day stipulated deadline by the Data Act?
4. What percentage of the total SARs received by your Trust were met but only after:
(a) subsequent complaint or verbal or written reminder for you to comply *after* the expiry of the stipulated deadline or
(b) threat of court action for your Trust's non-compliance.
(c) how many SARs were complied with *under* the stipulated forty-day maximum time for compliance?
5. Subject Access Request Awareness training:
(a) What training has your Trust's staff received regards their awareness of Subject Access Requests, and of their individual or collective legal obligations when being presented with a SAR?
(b) Who is (or was) your Trust's training officer or manager in charge of presenting that SAR Awareness training?
6. Who of your Trust is responsible for the receipt of and complying with SARs?
