Dear Wye Valley NHS Trust,

I've spoken with a person who badly injured their ankle by tripping over the shallow pavement edge in Widemarsh Street, Hereford. While in casualty at the County Hospital it was noticed that the scene of the accident was entered on the patient's notes.
How many people have been treated in casualty with injuries arising from falls or near falls in Widemarsh Street or similarly pedestrianised areas in the city of Hereford?

Yours faithfully,

Christopher Whitmey

Dear Sir

Thank you for your request for information.

Wye Valley NHS Trust responds that despite an in depth investigation into this request we are unable to offer the information without a manual trawl of patient notes. We can find no evidence that these statistics are kept separately and in an easily retrievable manner.

We estimate that the cost of complying with your request would exceed the appropriate limit of £450. The appropriate limit has been specified in the Freedom of Information and Data Protection (Appropriate Limit and Fees) Regulations 2004 and for healthcare services it is set at £450. This represents the estimated cost of one person spending 18 hours in determining whether the Trust holds the information, and locating, retrieving and extracting the information.

Under section 12 of the Freedom of Information Act the Trust is not obliged to comply with your request and we will not be processing your request further.

If you have queries or any concerns contact details are given at the top of the letter. Please remember to quote the reference number above in any future communications.
