I would like the following information:-

A) I request to see the appropriate policy/procedures covering 2015/2016 and 2017 for The clinical monitoring of patients who have been prescribed controlled drugs.

B) I also request - standard operating procedures for Ward Pharmacy Service to include class 3 drugs policy prescriptions for 2015/2016 and 2017.

C) I also want to know about Procedures/Policy specifically related to Controlled drugs procedure, relating to inpatients covering prescribing, dispensing, administering, recording and inpatients clinical monitoring of these prescribed and administered controlled drugs.

The requests were as follows and dont seem to be answered.
A ) I request to see the appropriate policy/procedures covering 2015/2016 and 2017 for Thw Clinical Monitoring of patients who have been prescribed controlled drugs, this does not appear to be supplied
B) I also requested - standard operating procedures for Ward Pharmacy Service to INCLUDE class 3 drugs policy prescriptions for 2015/2016 and 2017, this does not seem to be covered in the documents supplied
C) I also want to know about the Procedures/policy specifically related to controlled drugs procedure relating to in patients covering prescribing, dispensing, administering, recording and inpatients clinical monitoring of these prescribed and administered drugs.

The Medicines Management document you sent through is not clear which document is supposed to cover which part of myearlier request, please would you identify each question in reltation to request.

I cannot see anywhere in the documents you provided any information about controlled drugs procedures and cannot see any documentation with this name or any related information.

If you could supply everything I have asked for previously and now together the identification which document relates to each part of my request.

I am writing to request an internal review of Kingston Hospital NHS Foundation Trust's handling of my FOI request 'Standard Operating Procedure'.

I am still awaiting a response from my previous requests which are now long overdue, I am very upset that my questions have not been answered to date and when information has been sent it was incorrect and not the information I asked for. By law, under all circumstances, the authority should have responded by now.
