Response to this request is long overdue. By law, under all circumstances, Oxleas NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

Dear Oxleas NHS Foundation Trust,

How many patients have died while being cared for by Oxleas in the last 10 years? Can you please break this down month by month and what caused the deaths of the patients?

Yours faithfully,

Robin Cottle

Dear Diane Parsons,

I mean both - deaths on premises and deaths while on open referral. If you could break those figures up that would be great.

Yours sincerely,

Robin Cottle

Diane Parsons,

Dear Sir/Madam

Re. Request under the Freedom of Information Act 2000.

Thank you for your email received into our Trust 6th June 2013 requesting information regarding the number of patients that have died whilst under our Trusts care in the last 10 years, which I confirm is not held by our Trust. Natural causes deaths are not always reported, as strictly speaking they are not recordable incidents. We can provide data on the number of deaths reported but must be clear that this is a reported only figure.

We can also provide the number of deaths by month, however adding a cause of death ie. that on the death certificate will not be possible as we do not hold this information. We only collect whether the death was natural causes or various other coroner's verdicts.

Please advise if you wish to proceed with your request for the information that is held by our Trust.

Kind regards
