Special Allocation Scheme

The request was successful.

Dear NHS England,

I have been away from NHS GP as my Poplar GP removed me from his practice with out good reason, so he was no obliged to write me a private prescription for Human Growth Hormone replacement injections.

I complained about this breach of the code.

The NHS concocted a lie claiming the police were called to his surgery.

I have ten years later gone to the NHS Earls Court to see a GP.

This has alerted Primary Care Support England who fasley claim that I was threatening or aggressive in the surgery ten years ago and a hearing was held finding this so .

I was unaware of this public hearing.

I never went to the practice over Xmas. I called him by phone while on holiday. He couldn't face me after saying HGH is a forgery and conspired with an endocrinologist to prevent him signing a private prescription so I could buy Genotropin.

Please send me the police date, number and report for the call out, required to remove me from his list of patients.

Please send me all the department correspondence internally that allows them to make this decision on hearsay of a GP.

Please start a compliant again against the GP for not having any evidence required to remove me.

Please start a new public hearing before an independent and impartial tribunal within reasonable time, the presumption of innocence, to put my GP's false libellous allegation that the police were called to his surgery ten years ago to be examined as lies.

I should not be penalized by decisions affecting my rights or legitimate expectations unless I have been given prior notice of the case, a fair opportunity to answer it, and the opportunity to present my own case. Three rights were denied me ten years ago.

I will require both doctors to be present for my barrister Micheal Shrimton to question.

As soon as you realise I suffered injustice, please remove this impediment from my file and send me an appropriate ammount of money in Damages having to pay £300 a visit to a private Dr for ten years and decide if my GP and department officials decided to lie and libel me and settle my damages around£70,000.

I request all related documents and every ones names and home addresses and phone number to report to the police for the crime of conspiracy.

NHS England does not hold information in relation to your request.

It may help if we first explain that NHS England is not the same as ‘the
NHS in England’. The NHS in England is not a single organisation but is
made up of a range of organisations such as Hospitals, Clinical
Commissioning Groups (CCGs), and Ambulance Trusts. Therefore, any recorded
information NHS England may be able to provide in response to an FOI
request will generally relate to the services NHS England commissions.

Accordingly, NHS England does not hold information in relation to practice
staff, their contact information or any practice-level correspondence
related to individual patient removals. This information may be held by
the individual practice. [1]Practice contact information can be found on
the NHS.uk website.

NHS England are unable to comment on information that may be held as part
of individual patient records; as they are confidential, we do not have
access those records. As such, you may wish to submit a Subject Access
Request (SAR) to the surgery. Information on [2]how to make an SAR can be
found on the Information Commissioner’s website.

Information about [3]procedures for adding patients to the Special
Allocation Scheme can be found on the Primary Care Support England (PCSE)
website. Please note that PCSE is not responsible for determining if a
patient meets the criteria for immediate removal, other than confirming
the incident has been reported to the Police.  This is the responsibility
of the reporting practice.

Under the NHS Complaints regulations, complaints can be made to either the
commissioner (the organisation that pays for the service, in this case NHS
England) or the provider (in this case, the GP practice) of the relevant
service. Details on [4]how to complain to NHS England can be found on our
website.

Information about claims against NHS England can be sought from [5]NHS
Resolution. Additional guidance may be available from [6]Citizens Advice,
either via their website, or in person.

We hope this information is helpful. However, if you are dissatisfied, you
have the right to ask for an internal review. This should be requested in
writing within two months of the date of this letter. Your correspondence
should be labelled “Internal Review” and should outline your concerns
