Dear Jack Allerton

I refer to the above and your request dated 25, April 2018. I have
contacted the relevant services and now provide a response to the
following request regarding social services team members and contact.

What I would like from you is a list of ALL of the INDIVIDUAL TEAMS within
Children's and Young Peoples social Services, also including the NAMES and
CONTACT NUMBERS of ALL TEAM MANAGERS.

Information relating to CYPSS team managers is available in the form of an
organisational structure.  The structure is in the process of being
updated and once it is finalised, we will send a copy to you. 

Also ALL of the INDIVIDUAL TEAMS within Adults, older people and mental
health social Services.

Also including the NAMES and CONTACT NUMBERS of ALL TEAM MANAGERS

Please see attached spreadsheet which provides the names and teams for
adults and health managers.   The contact number for all adults and health
managers is 0113 222 4401.

I trust that this response is satisfactory. If however you are
dissatisfied with this response you have a right to appeal to an
independent officer within Resources and Housing. They will re-examine
your request and assess whether we have adhered to the requirements of the
act. Should you wish to follow this course of action please address your
