Social Prescribing:

The details we require are:

1. Is a digital social prescribing platform already procured in your locality?

a. If the answer is No, are there any plans to procure a digital social prescribing platform in 2023/2024?
b. If the answer is Yes, can you provide the name of the digital social prescribing platform that has been contracted?

2. Actual spend per annum of the contract?
3. Start date and end date of the contract?
4. Is there an extension clause in the contract? And if so what is the duration?
5. Were implementation milestones met in a timely manner? And what was the duration of fully implementing the digital social prescribing platform?
6. Has a decision been made yet on whether the contract is being either extended or renewed?
7. Who is the senior officer/named lead (outside of procurement) responsible for this contract?
8. Can you provide details of the providers in your locality commissioned to deliver a social prescribing service?
