Dear Derbyshire County Council,

I am writing to request an internal review of Derbyshire County Council's handling of my FOI request 'Social Care Forms'.

I am requesting an internal review of my FOI request because there is hardly any of the Children Social Care template forms you use on http://derbyshirecaya.proceduresonline.c... so I would like this provided.

2. Adult Social Care Services- Care Management & Reablement Service Forms and referral and case-closure forms.

Unfortunately we haven’t got the forms saved so it would involve going through every one and printing off. So far, one member of staff has spent 3 hours to identify, retrieve and save 26 Case Management documents there are approximately 400 relevant documents which would take us up to 58 hours.

I would like it if you could send me the Reablement Assessment Form, the reablement care-plan, social care assessment report, social care care plan and the case-closure form and referral record specifically instead then as I know this wouldn't take 18 hours to complete.
