Sinking of Patafea Pecosa

Dear Canal & River Trust,

I refer to your Customer Service Standards (minimum safety standards) and also your previous standards adopted from British Waterways. Both state -

'Lock chambers and gates are free of protrusions / indentations that could snag boats. Gates are fendered.'

I also refer the incident where narrowboat Patafea Pecosa sank on 10th August 2012 at Bank Newton Lock (40) on the Leeds & Liverpool Canal after hanging up on a lock wall protrusion -

Please provide the following information -

1) Annual Inspections(AI) reports over the last 10 years.

2) Principal Inspection (PI) reports over the last 10 years.

3) The latest Lock Gate Inspection (LGI).

4) The latest Length Inspection (LI).

5) Any 'near miss' (or similar incident reports) related to boats hanging up in this lock over the last 10 years.

6) Any recorded information related to the Lock 40 site visit in June 2013 by Tony Stammers.

7) Any recorded communication between the Trust and HSE related to the incident.

8) Any recorded information that indicates remedial action to repair the lock wall would be taken.

9) Any recorded information that indicates that such work was carried out.
