Dear Operations FOI Requests,

Thank you for the JSA information.

The UC self-certification information was not helpful. Can you provide the related leaflet(s) and form(s) please?

Self-certification for sickness
A period of self-certification for sickness covers up to the first 7 calendar days of sickness. After 7 calendar days a Statement of Fitness for Work (fit note) is needed.
