Thank you for your recent Freedom of Information Request, which we received on 30 December 2014.

Please find responses to your specific questions set out below:

Westminster City Council shares the following services with the London Borough of Hammersmith and Fulham and the Royal Borough of Kensington and Chelsea under Tri-Borough arrangements:

Children’s Services

We list the services where there are joint teams involving social care, and other local authority children’s services, some of which are jointly funded, and some not. These operate across the Tri-Borough.

• Early years services
• Early help/localities teams
• Multi-Agency Safeguarding hub
• Specialist HVs and Child and adolescent mental health professionals in social work teams
• Camhs and OTs in Children with disabilities teams; social worker in Community Paediatrics
• Looked After Children nurses co-located with social work teams
• Family recovery teams
• Integrated gangs unit (Westminster only)
• Substance misuse teams
• Youth Offending Service

The strategy has been to analyse the need for multi-disciplinary activity and the best way to provide it, either through shared services, through co-location or through close liaison between specialist teams. In some cases, this has led to the development of multi-agency teams such as the early intervention teams, made up of a wide range of different professionals, supporting families in an area, working with schools, primary care and children’s centres. Similarly, to address the needs of troubled families, there are tri-borough multi-agency services. This work continues as each service is reviewed. There are also plans to integrate health services vertically, with a focus on primary care, which will encourage greater integration with local authority services.

Adults Services

Currently, all Care Management Teams are geographically based and operate on a borough by borough basis. Strategic management and commissioning and support functions, however, operate on a Tri-Borough basis. This includes the following services:

• Procurement and Contracting
• Workforce Development
• Business Analysis
• Planning, Service Improvement and Projects
• Community Commissioning
• Complex Needs Commissioning
• Professional Standards and Safeguarding
• Community and Residential Finance
• Client Affairs
• Social Care Case Management IT System

2. Please describe your plans with joint working between Social Care and Health.

The Tri-Borough Local Authorities have partnership agreements in place with each of the relevant Clinical Commissioning Groups and a programme of jointly commissioned services for older people, younger adults with learning disabilities, physical disabilities and mental health problems.

They are currently working together to develop Better Care Fund plans for 2014-15 and 2015-16 in accordance with government requirements. These are building on current services and our knowledge of the needs of our residents to increase integration of health and social care, particular in the areas of: rehabilitation and reablement, community independence services, long term conditions with the aim of providing care and support to people as close to home as possible.

The Tri-Borough Better Care Fund draft plan has been submitted and circulated widely as an example of good practice. Detailed proposals are now being developed for consideration by the three Health and Wellbeing Boards.
