Freedom of Information Act 2000 - Request for Information

I am writing in respect of your recent enquiry for information held by the Authority under the provisions of the Freedom of Information Act.

Please find the information you requested.

Q1 Do you currently have any shared services within Social Care with other/s Local Authorities?
If yes, please state with who and what areas e:g Children’s, Adults etc…

In adult social care, the only service we jointly commission is the Independent Mental Capacity Advocacy service, which is jointly commissioned with Kirklees.

The Council and NHS Wakefield CCG jointly commission a Volunteer Befriending Service, the WeSAIL support service, Speech and Language Therapy services and a Young Person’s Bereavement Counselling response as well as a number of individual bespoke packages of care for children and young people with complex needs. The Council works with other local authorities in West and South Yorkshire within the White Rose Procurement Collaborative to commission Independent Foster Agencies and Residential Placements for children and young people and are currently working to widen the range of services covered by this arrangement.

Q2. Please describe your plans with joint working between Social Care and Health

There is a joint programme of transformational change in place across the Council, Clinical Commissioning group and NHS providers in Wakefield. This includes planning around urgent care, maternity, children and young people, mental health and care closer to home, with details of care closer to home below.
Care Closer to Home
Wakefield Council and Mid-Yorkshire Hospitals NHS Trust are developing arrangements to:

• Consolidate the Council’s Adult Operations Social Care Services with the NHS community services arm of the Mid-Yorkshire Hospitals NHS Trust.
• Although there will be closer integration of some existing services this will not require the creation of a separate organisation or the complexities of a TUPE transfer
• Some services will continue to be provided separately by the Local Authority or Health, but others will be delivered by multi-disciplinary teams co-located whenever this provides an advantage
• The aim of these arrangement is to provide service users and patients with greater choice closer to home and to reduce the need for more complex and expensive care packages, unnecessary hospital admissions and to facilitate improved hospital discharge arrangements to prevent unnecessary delays of people being discharged home.
• A joint post of “Director of Integrated Care” has been appointed into the post in order that the transformation and integrated changes above can be taken forward.

Working Aged Adults.
For a number of years, we have had partnership agreements in place between the Council and CCG in respect of lead commissioning and pooled budgets. We have a number of joint arrangements in place, for example, partnership boards and joint commissioning boards and have a joint assurance process in place for Winterbourne recommendations. We have integrated community teams for people with learning disabilities and people mental health problems.

We jointly fund and commission preventative services, for example Well Women and Carer support.

Children and Young People’s Services
The Council and NHS Wakefield CCG along with other partners within the local Children and Young People’s Partnership have agreed in principle to work towards the integration of services which support children, young people and families. Initial work is progressing in relation to early years and early intervention services including children centres, health visiting, school nursing etc. In response to the Government’s Special Educational Needs and Disability agenda partners are working together to build on existing jointly commissioned services and close working relationships in other services to create an integrated response.
