I refer to your request below. I can give you the following information.
Children's services:
1. Do you currently have any shared services within Social Care with other/s Local Authorities?
If yes, please state with who and what areas e:g Children’s, Adults etc…
a) For Children's services:
No
b) For Adults services:
Currently within Adult Social Care Services there is only one post shared with Manchester Council.
This is The Health Manager who manages an integrated team of Social Workers, District Nurses and Discharge Facilitators.
Please find attached a description of how the team works.
2. Please describe your plans with joint working between Social Care and Health.
a) For Children's services:
Trafford Children and Young People's Service is a partnership dedicated to making a difference to the lives of children, young people and their families in the Borough. Staff from the Council and Health are co-located and managed in area based family support teams delivering education, health and social care services in four localities across the Borough. A Section 75 Agreement exists between the Council and the Trafford Clinical Commissioning Group (CCG) to commission and deliver services. The Children's Trust Board and Joint Commissioning Group identify the resources, priorities and plans to ensure that Trafford is an outstanding place for people to live, learn, work and relax where all children and young people feel and are safe from harm, are healthy and are receiving an outstanding education preparing them for adult life.
Trafford Council has a number of section 75 Partnership Agreements in place with externally commissioned health services which detail the arrangements for the provision of integrated social care and health services, specifically agreement exists for adult health and social care between TMBC and Pennine NHS Foundation Trust, and for children and young people's health, social care and wider children's services between TMBC, Pennine NHS Foundation Trust and Central Manchester Foundation Trust.

b) For Adults services:
The Ascot House Assessment and Rehabilitation Unit is joint working with Health and Social Care, I have attached the document which explains how this unit operates.
The unit also has an office unit with integrated teams from health and social care.
The Social Care Staff comprise:- Rapid response teams, Social Care Assessors, Out of hours Social Work Team, Learning Disability Supported Living Manager
The Health Staff comprise:- Community Rehabilitation Team, out of hours District Nurses, I.V Team, Urgent Care Team.
There are plans to integrate with health in other areas but these have not yet been identified.
