Dear Healthcare Improvement Scotland,

I am writing to you under the Freedom of Information Act 2000 to request the following information from the Human Resources Department:

1) number of FTE Death Certification Review Service staff as at 16/11/2017, by role.
2) Death Certification Review Service structure as at 16/11/2017, by role.
3) number of FTE Death Certification Review Service staff as at 16/11/2018, by role.
4) Death Certification Review Service structure as at 16/11/2018, by role.

Please provide the information in the form of a letter.

If it is not possible to provide the information requested due to the information exceeding the cost of compliance limits identified in Section 12, please provide advice and assistance, under the Section 16 obligations of the Act, as to how I can refine my request.

If you can identify any ways that my request could be refined I would be grateful for any further advice and assistance.

Dear Healthcare Improvement Scotland,

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of Healthcare Improvement Scotland's handling of my FOI request 'Service Structure'.

I believe the organisation has incorrectly reported the Operations Team Manager role in the November 2017 service structure as ‘Operations Team Lead’.

I believe Healthcare Improvement Scotland has attempted to generalise the ‘operations Team Lead’ role within both the 2017 and 2018 service structures to conceal information.

I would be grateful if you could investigate and confirm the specific job title of the ‘Operations Team Lead’ role stated in the November 2018 service structure.

In addition to my original request, i would be grateful if you could provide the pay Band for all roles on both the 2017 and 2018 service structures.
