Dear Sir/Madam

Further to your recent FOI request, which you submitted to the Council on 21 June 2017. I am contacting you to request that you provide clarification as to the exact nature of the information you require. My contact in the department would like clarification as to whether you are asking about the care management process and user risk assessments done as part of a care assessment, or are you asking about particular contracts, e.g. home care, nursing and residential placements, etc?

Once you have provided the requested clarification - I will send you a revised date for providing you with the information.

Regards

Dear Yu, Robin: CP-ICT: RBKC,

It beggars belief that on 23 June 2017, I was asked to clarify as to whether I am asking about the care management process and user risk assessments done as part of a care assessment, or are you asking about particular contracts, e.g. home care, nursing and residential placements, etc, further indicating that you are fully aware and familiar with both items for which you required clarification AND AFTER I clarified what information I am after, now you ask for an example..despite the fact that it is crystal clear what I am after.

Please provide information regarding the care management process and user risk assessments done as part of a care assessment.

Yours sincerely,

A Zaimes

Dear Yu, Robin: CP-ICT: RBKC,

I do not have any examples as it is the first time I ask for this information. Please provide a generic service contract & terms of service as well as generic risk assessments.

Yours sincerely,

A Zaimes
