Dear West London NHS Trust,

Please could you inform us- how many SUIs ( serious untoward incidents ) have occurred in the last 5 years in your Trust?

Please classify them further
1] as suicide/ homicide/ others

2] in which areas these occurred ---Ealing/ Hammersmith/Hounslow

3] how many of these SUIs were in the in-patient department and how many were in the community.
