How many of those serious incident reports have undergone route cause analysis?

How many of those serious incident reports have been due to poor standards of care, medical errors?

How many of those serious incident reports resulted in avoidable deaths?

Dear Ms Tomlinson,

Please see attached the 2 individual FOIs the Trust has received from Emma Tomlinson regarding Serious Incidents and fluids. These are referenced as 483-22, and 484-22.

Freedom of Information guidance states that multiple requests received within 60 consecutive working days with the same or similar topic and from one person or company can be aggregated, with regards to the cost of compliance.

The Trust is therefore aggregating the above mentioned 2 requests and will be responding accordingly.

I am writing to request an internal review of Manchester University NHS Foundation Trust's handling of my FOI request 'Serious incidents reports'.

I note that you have aggregated my requests, which authorities are permitted to do for the purposes of a Section 12 exemption — but as you may be aware, ICO guidance (https://ico.org.uk/media/for-organisatio...) states that this practice allows "the aggregation period to only run up to 20 days ‘forward’ from the date of any single request under consideration" and "up to 20 days ‘backwards’ from the date of any single request under consideration" and "the total aggregation period (running either forwards or backwards or a combination of both) from the date of any single request must not exceed 60 working days".

For this reason I believe the exemption is invalid when applied in this manner, and request that you review your response.

In response to your query the Trust has sought advice from the ICO
regarding their guidance on aggregation of multiple of FOI requests.
Following this, the FOI Team can confirm that we are satisfied that the
aggregation guidance has been correctly applied in this instance.
Therefore, your FOI requests referenced 483-22, 484-22 and 485-22 will
remain aggregated in terms of the cost of compliance as previously stated
and will be processed as such.
