Further to your FOI dated 10 Oct 2018, I can now provide you with the
information below and attached.
•Please can you share your serious incident reporting criteria and process
We follow the NHS England Serious Incident Framework and we have an
internal policy to support this – please see attached
•Are all patients that are admitted to a caseload who die unexpectedly
reported as a serious incident for the organisation?
If they meet the criteria of the Serious Incident Framework – otherwise we
review each case in our Patient Safety Team and determine any other action
to be taken.
•If not – what criteria do you use?
If you have any concerns about the way your request has been handled; the
response received, or any ‘exemption’ decision you should, in the first
instance contact me to try to resolve them. If you are not happy with the
outcome you can write to the Trust Secretary at the address below and make
a request for an internal review.
